Hall of Fame Nomination Form

Any alumni wishing to nominate a candidate for induction must fill out the form below and submit it to the LHS Alumni Association before July 1st.

Any submissions after July 1st will be added to the following year’s submissions.

Previous nominations must be resubmitted each year.

The requirements for eligibility are as follows:

  1. The candidate must have graduated from Leesville High School
  2. The candidate must have graduated at least 10 years prior to nomination
  3. The candidate must have played at least one year of any varsity sport

The person making the submission will also include a short bio of the candidate to include some of the candidate’s accomplishments while at LHS. If the candidate went on to play in college or professionally, that may also be included.
